Position Overview
Monogram Health is seeking an experienced Palliative Care Nurse Practitioner to deliver in-home care to a highly complex patient population in the Newark, NJ market.
This is a permanent, full-time position focused on end-of-life and serious illness care. The NP will provide comprehensive palliative services, including pain and symptom management, advanced care planning, and coordination with interdisciplinary teams.
This is not a contract role — it is a long-term, permanent opportunity.

• Monday – Friday
• 8:00am – 4:30pm (flexibility based on patient scheduling)
• No call
• No weekends

Patient Model
• In-home visits (must be comfortable traveling locally)
• Travel radius up to 90 minutes within market
• Most patients clustered within core territory
• Average 3–4 patient visits per day
• Visit duration may extend up to 90 minutes

• End-stage renal disease (ESRD)
• Diabetes
• COPD
• CHF
• Advanced chronic illness

Responsibilities
• Conduct in-home palliative assessments and evaluations
• Develop and manage individualized care plans
• Manage pain and complex symptom control
• Guide patients and families through goals-of-care discussions
• Document advanced directives
• Collaborate with interdisciplinary team (RN, Social Worker, Chaplain)
• Coordinate with primary and specialty providers

Minimum Requirements
• Active New Jersey NP license
• Active, unrestricted DEA
• Board Certified NP (AANP or ANCC)
• Minimum 2 years recent palliative care experience (required)
• Ability to practice autonomously
• Comfortable with home-based care model
Preferred:
• AGACNP, AGNP, or FNP
• Experience working with nephrology/ESRD patients
