Hospice Nurse Practitioner – Pediatric Hospice Care (Part-Time, Travel Assignment)

Join our compassionate team as a Hospice Nurse Practitioner dedicated to providing exceptional end-of-life care to pediatric patients. Based out of our Fresno location, this flexible, part-time travel assignment is perfect for healthcare professionals committed to delivering compassionate hospice care.

What You Will Do:
  • Conduct home visits (4–5 per day) to deliver comprehensive hospice care in a compassionate manner.
  • Provide care in skilled nursing facilities (SNFs) and care facilities as needed.
  • Perform face-to-face evaluations for Medicare recertifications in collaboration with the Medical Director.
  • Communicate effectively with Case Managers to coordinate patient care and follow-up.
  • Participate in inpatient rounding when necessary to ensure continuity of care.
  • Educate and support clinical team members on hospice best practices and patient needs.

Required Skills:
  • Valid Nurse Practitioner (NP) license with pediatric hospice experience.
  • Strong clinical assessment and evaluation skills.
  • Excellent communication and teamwork abilities.
  • Ability to work independently, prioritize tasks, and adapt to a flexible schedule.

Nice to Have Skills:
  • Experience in hospice or palliative care beyond pediatrics.
  • Familiarity with Medicare recertification processes.
  • Bilingual capabilities to serve diverse patient populations.

Preferred Education and Experience:
  • Master’s degree in Nursing (MSN or higher).
  • Minimum of 2 years’ experience as a Nurse Practitioner, preferably in hospice or pediatric end-of-life care.

Other Requirements:
  • Valid licensure and certifications in good standing.
  • Travel reimbursement and licensing assistance provided.
  • Availability for 1–2 days per week, with flexible scheduling (full or half days).
  • No evening or weekend shifts required.

Hourly Pay Rate: $81.74
