Palliative Care Nurse Practitioner Needed – Southern Oregon Region
Start Date: February 2026
Contract Length: Ongoing
Schedule
Full-time scheduled clinical hours
Monday–Friday, 8:00a–5:00p (1-hour unpaid lunch, flexible timing)
Back-up call support provided for a secondary regional site via telehealth only
Weeknight call: 4 nights per week (5p–8a), beeper
Weekend call: potential 1 weekend per month (8a–8a)
Final call schedule set after confirmation
Practice Details
Outpatient palliative and hospice care service
Onsite patient management with remote support responsibilities
Support staff available
EMR: Epic
Clinical Scope
Admissions and recertification face-to-face visits
Primary responsibility for symptom management and medication prescribing
Clinical collaboration with RNs for patient needs
Management of palliative/hospice patients across a broad range of diagnoses
Requirements
Nurse Practitioner – Palliative Care
Board Certified
Active Oregon and Washington licenses required
DEA for both WA and OR required
BLS required
Prior palliative or hospice experience strongly preferred
Must be organized, responsive, and timely for credentialing
Credentialing
Approx. 30–90 days
Start date dependent on file completion
No name clear required
Submittal must include NPI and full license details
Location
Located in the Southern Oregon Region, supporting an outpatient palliative and hospice service with full-time weekday hours, symptom-management responsibilities, and structured weeknight/weekend call rotation.
