Hospice Nurse Practitioner (Part-Time, Travel-Based, Palliative Care)

Are you an experienced Nurse Practitioner passionate about delivering compassionate hospice and palliative care? Join our dedicated team in a flexible, home-based hospice role, supporting patients in Fresno and surrounding areas. This is an excellent opportunity to make a meaningful difference while enjoying a adaptable schedule tailored to your availability. As a key clinical partner, you will assist in recertification evaluations, coordinate patient follow-ups, and possibly conduct home visits or inpatient rounds. Required Skills:
  • Valid Nurse Practitioner license with Palliative Care certification
  • Pediatric experience in a clinical setting
  • Strong clinical assessment and evaluation skills
  • Excellent communication and interpersonal abilities
  • Ability to coordinate with a multidisciplinary team
  • Proficiency in documentation and Medicare recertification processes

Nice to Have Skills:
  • Prior hospice or home health experience
  • Familiarity with hospice software and electronic health records
  • Experience working with diverse pediatric populations
  • Ability to adapt to flexible schedules and travel needs

Preferred Education and Experience:
  • Master's degree in Nursing (MSN or higher)
  • Minimum of 2 years of clinical experience, preferably in hospice, palliative, or pediatrics settings
  • Valid state license and current certifications in CPR, BLS

Other Requirements:
  • Travel flexibility to visit patients at homes, skilled nursing facilities, or care centers
  • Reliable transportation and valid driver’s license
  • Willingness to work 1-2 days per week, with schedule flexibility for full or half days
  • Ability to communicate effectively with patients, families, and healthcare teams
