Required Skills:
- Valid Nurse Practitioner license with Palliative Care certification
- Pediatric experience in a clinical setting
- Strong clinical assessment and evaluation skills
- Excellent communication and interpersonal abilities
- Ability to coordinate with a multidisciplinary team
- Proficiency in documentation and Medicare recertification processes
Nice to Have Skills:
- Prior hospice or home health experience
- Familiarity with hospice software and electronic health records
- Experience working with diverse pediatric populations
- Ability to adapt to flexible schedules and travel needs
Preferred Education and Experience:
- Master's degree in Nursing (MSN or higher)
- Minimum of 2 years of clinical experience, preferably in hospice, palliative, or pediatrics settings
- Valid state license and current certifications in CPR, BLS
Other Requirements:
- Travel flexibility to visit patients at homes, skilled nursing facilities, or care centers
- Reliable transportation and valid driver’s license
- Willingness to work 1-2 days per week, with schedule flexibility for full or half days
- Ability to communicate effectively with patients, families, and healthcare teams
