Job description
UChicago Medicine AdventHealth is hiring for their Community Palliative Care program. This is a growing program which is so needed in our communities. The program serves our homebound patients, giving them access to palliative care in the comfort of their homes.
· 1-3 years of APP experience, preferred
· Palliative Care Certification, preferred
· Weekly Monday - Friday schedule
· No call requirement
· Outpatient palliative care in patient’s homes
· Visit patients in assigned territory
· Work collaboratively with Social Workers and House Calls Physicians & APP's
· Competitive compensation, benefits, CME and occurrence-based malpractice insurance
· Annual Salary: $110K - $120K
REQUIRED QUALIFICATIONS:
Education/Training
- Graduate of an accredited school of nursing and nurse practitioner program
- Masters degree in Nursing or Physician Assistant Studies
- Current Illinois license in good standing
- Certified as a nurse practitioner or physician assistant by an approved certifying organization.
- Prescriptive authority, Illinois Controlled Substance, DEA certification
- Current CPR certification
