What You’ll Do:
Asante is seeking a compassionate and dedicated Nurse Practitioner to join our Outpatient Palliative Care Team. Based in the heart of Southern Oregon, this position offers the unique opportunity to provide holistic, patient-centered care to individuals with serious or life-limiting illnesses—in the comfort of their homes.

What You’ll Do:
- Deliver comprehensive palliative care in an outpatient and home-based setting.
- Perform thorough patient assessments, develop and implement individualized care plans, and provide symptom management.
- Prescribe medications, order and interpret diagnostic tests, and provide counseling and education to patients and families.
- Collaborate closely with a multidisciplinary team, including physicians, nurses, social workers, chaplains, and other healthcare professionals.
- Support patients’ physical, emotional, spiritual, and social needs during some of the most critical times in their lives.
- Participate in quality improvement, education, and research initiatives to enhance palliative care services.
- Autonomous, meaningful work with strong team collaboration.
