Palliative Care Nurse Practitioner - Jacksonville, NC

Job Description:

We are seeking an experienced and dynamic Nurse Practitioner to build and lead our palliative care presence. This unique role combines direct patient care with program development. The ideal candidate will be a seasoned professional comfortable with both clinical excellence and community networking.

Key Responsibilities:

  • Provide palliative care to patients in various settings: personal residences, assisted living facilities, and skilled nursing facilities.

  • Manage a caseload with a goal of 20+ visits per week (averaging 4-5 visits per day), with flexibility to adapt to patient needs.

  • Conduct a weekly palliative care clinic on Wednesdays (8 AM - 12 PM).

  • Educate patients and caregivers on disease management, advance care planning, medication administration, and end-of-life expectations.

  • Coordinate care with community medical providers and other disciplines to ensure seamless patient care.

Qualifications:

  • Graduate of an accredited school with a Master's Degree in Nursing.

  • Current certification from the AANP or ANCC.

  • Current (or ability to obtain) licensure from the NC Board of Nursing (NP).

  • Minimum of five years of general medical experience (inpatient, outpatient, nursing home).

  • Minimum of four to five years of experience as a Nurse Practitioner.

  • Palliative care experience is required.

  • Certification in Hospice/Palliative Care.

  • Experience seeing patients in a home setting and Hospice experience (as a complement to palliative care)
