Palliative Care Nurse Practitioner

Practice Overview

  • You'll join a well-established palliative care team focused on patient-centered, community-based care
  • Lead a weekly outpatient palliative care clinic every Wednesday morning
  • Conduct in-home visits and develop relationships with referring partners across the county


Schedule

  • Monday through Friday, 8 AM to 5 PM schedule
  • No weekend or on-call responsibilities
  • Dedicated time for clinic, home visits, and outreach initiatives
  • Flexible structure supporting both clinical care and strategic program development


Compensation & Benefits

  • Structured base pay with strong potential for performance-based growth

Candidate Requirements

  • At least 4-5 years of Nurse Practitioner experience required
  • Must have a strong background in palliative care
  • Comfortable with community outreach, public speaking, and program development
  • Hospice experience is a plus, but not required
