A compassionate and collaborative healthcare organization in Northwest Ohio is seeking a full-time Board-Certified Nurse Practitioner to join its dedicated Outpatient Palliative Care team. Position Highlights:

Deliver outpatient palliative care in a variety of settings:

Home visits
-
Extended care facility consultations
-
Telehealth
-
Clinic-based care

Manage 6–8 patient visits per day, depending on location across a 9-county region
-
Lead sensitive goals-of-care discussions with patients and families
-
Evaluate, plan, and coordinate individualized care aligned with palliative care standards

Partner with a collaborative team of NPs, physicians, and support staff
-
Engage in transitions of care and identify when hospice services may be appropriate

Hospitalist team is responsible for patient admissions
-
Maintain thorough documentation and strong communication with care teams

Reliable personal transportation required (paid mileage; no vehicle provided)
Candidate Requirements:
-
Board-Certified Nurse Practitioner – REQUIRED
-
Ohio license (or willingness to obtain) – REQUIRED
-
Experience in palliative care, hospice, or acute care settings – REQUIRED (no exceptions)

Clean malpractice and license history – REQUIRED
