Nurse Practitioner – Outpatient Palliative Care | Findlay, Ohio

Schedule: Full-Time | Monday–Friday

A compassionate and collaborative healthcare organization in Northwest Ohio is seeking a full-time Board-Certified Nurse Practitioner to join its dedicated Outpatient Palliative Care team. This is an opportunity to make a lasting impact on patients and families facing serious illness by providing thoughtful, goal-directed care in a supportive, interdisciplinary environment.

Position Highlights:

• Deliver outpatient palliative care in a variety of settings:

  • Home visits

  • Extended care facility consultations

  • Telehealth

  • Clinic-based care

• Manage 6–8 patient visits per day, depending on location across a 9-county region

• Lead sensitive goals-of-care discussions with patients and families

• Evaluate, plan, and coordinate individualized care aligned with palliative care standards

• Partner with a collaborative team of NPs, physicians, and support staff

• Engage in transitions of care and identify when hospice services may be appropriate

• Hospitalist team is responsible for patient admissions

• Maintain thorough documentation and strong communication with care teams

• Reliable personal transportation required (paid mileage; no vehicle provided)

Candidate Requirements:

• Board-Certified Nurse Practitioner – REQUIRED

• Ohio license (or willingness to obtain) – REQUIRED

• Experience in palliative care, hospice, or acute care settings – REQUIRED (no exceptions)

• Clean malpractice and license history – REQUIRED
