Full-time Outpatient Palliative Care Nurse Practitioner Opportunity available in Northwest Ohio

  • Hospital employed position
  • Services including home visits, extended care facility consultations, telehealth, and clinic-based care.
  • Manage 6–8 patients per day based on location across a 9-county area in NW Ohio.
  • Hours are Monday-Friday 8a-5p, with no call or weekends
  • Participate in delicate, goals-of-care discussions with patients and families facing serious or terminal illnesses.
  • Assess, plan, implement, and evaluate individualized care needs in alignment with palliative care principles.
  • Collaborate with a supportive interdisciplinary team including other NPs, physicians, and administrative staff.
  • Participate in care transitions, including determining when patients would benefit from continued palliative care or hospice services.
  • Work closely with a hospitalist team responsible for all patient admissions.
  • Document patient encounters thoroughly and maintain excellent communication with patients, families, and care teams.

Qualifications

-Board Certified NP
-OH license (or willing to obtain) 
-Experience as advanced provider in an acute care setting or with palliative care and hospice
-Clean malpractice and license
