Position: Nurse Practitioner (NP) – Mobile/Travel Role
Location: ISNP – San Fernando Valley (Burbank → Santa Clarita → West Hills)
Start Date: ASAP
Duration: 6 months with extension options
Pay: Bid
Overview:
Seeking an experienced Nurse Practitioner to provide geriatric primary care to patients in their homes and facilities throughout the San Fernando Valley. This is a mobile/travel NP role focused on value-based care.
Schedule:
- Monday–Friday
- 8:00 AM – 5:00 PM
- No on-call
- Patient volume: 5–7 per day
Requirements:
- Active CA NP license
- Board Certified (BC)
- BLS & DEA
- Minimum 1 year geriatric primary care experience
- Experience visiting homes/facilities as a mobile NP
- Must have knowledge of HEDIS and Value-Based Care (VBC)
