Job description
We are seeking licensed providers to conduct in-home health assessments in Wisconsin. This is an assessment-only role focused on preventive care and chronic condition review — no prescribing or ongoing treatment responsibilities required.

Full-time (40 hours/week) or Part-time (20 hours/week) options available

Monday–Friday schedule

8:00 AM – 5:00 PM with a 1-hour lunch break

Schedules released 2–3 weeks in advance

1–3 hours of paid orientation
-
Ongoing education modules to be completed during downtime within working hours
Role Responsibilities

Conduct 5–6 in-home health assessments per day

Perform annual risk assessments and chronic condition reviews

Complete A1C testing when applicable (no additional screenings required)

Escalate urgent findings to the patient’s Primary Care Provider (PCP)

Mileage reimbursed
Qualifications
-
Active Wisconsin Medical License (required)

Must be enrolled and credentialed in Medicaid

Ability to travel within assigned county

Strong clinical assessment and documentation skills
