Home Care Opportunity – Nurse Practitioner - Orange County, CA
🩺 About the Role
Seeking a dedicated and experienced nurse practitioner for a home care position in Orange County, CA. This opportunity can either be a 6 month locum's contract or a permanent employment.
- Location: Orange County, CA
- Position Type: Full time
- Setting: Home care - Focus is on geriatric populations, especially those suffering from nephrology disorders and other poly-chronic conditions
- Start Date: Early November start date is expected
- Duration: 6 month's locums contract OR permanent employment
- Schedule: Monday-Friday 8a-5p
- Pay Rate: Competitive pay, W2 with included benefits
📋 Qualifications
• Board Certified NP
• CA state license
• Minimum 1 year of experience in home care and treating geriatric populations
• Strong communication and patient care skills

Job Responsibilities
• Conducts assessments, which includes comprehensive annual wellness exams on patients both in the patients' home and in the virtual environment
• Counsels and educates patients and families about benefits and programs available to help them live healthier lives
• Documents items such as: appropriate chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan
• Responsible for the coordination of care with primary care providers, specialists, and appropriate ancillary services
• Completes all documentation and paperwork in a timely manner
• Maintains quality of care standards as defined by the practice
• This position will not be office-based but will be remote in state in which employed and will need to attend periodic training/meetings outside of that state

Willing to cover at least a 1 hour radius

8am-5pm
