Start Date: June 24th
Duration: 90-day assignment
Location: Home visits across Ocean County - must live in Ocean County
Scheduling:
Must be available between 8 AM – 8 PM (Scheduled for an 8-hour shift within this window, e.g., 8 AM–5 PM or 12 PM–8 PM)
One-hour lunch break included
Schedules are released 2–3 weeks in advance
Full-time, 40 hours/week, Monday through Saturday (flexible within that window)
Job Details:
Conduct 5–6 home health assessments per day
Paid for a full day, regardless of patient volume
Perform annual risk assessments and review of chronic conditions
No treatment or prescribing – assessment-only role
Escalate urgent findings to the patient’s PCP
If applicable, conduct A1C tests only (other screenings left for the patient)
Travel required; mileage reimbursed daily
Equipment:
iPad
Blood pressure cuff
Lab kits for patient use
Credentialing:
1–2 week turnaround
Must have an active NJ license and NJ Medicare number
Qualifications:
Must be licensed in NJ
Mileage will be reimbursed daily
- if taking PA must be Board-Certified Physician Assistant
