IHP is looking to add an APP to our team as we expand our services to new facilities. IHP is a local, physician owned, provider management organization that offers inpatient care in Short term and long term hospitals, rehab facilities, post acute facilities and SNFs.
Basic Job Details:
- Looking for Compassionate, caring and talented APP to cater to the medical needs of seniors
- Monday-Friday, 8am-5PM or 9-6PM
- Call One in 3 weekends per month
- 14-16 patients per day
- Take care of the patient in post-acute care and long-term facilities
- EMR utilization/notes
Qualification:
- Board-certified APP, NP or PA
- 1 year Experience caring for the geriatric population and post acute care a plus
