Job description IHP, a local physician-owned provider management organization, is expanding its services to new facilities and is looking to add a compassionate, caring, and talented Advanced Practice Provider (APP) to our team. We offer inpatient care in short-term and long-term hospitals, rehab facilities, post-acute care facilities, and skilled nursing facilities

Position: Advanced Practice Provider (APP)
Schedule: Monday to Friday, 8 AM - 5 PM or 9 AM - 6 PM
Call Schedule: One in 4 weekends per month
Patient Load:14-16 patients per day
Responsibilities: Provide medical care to patients in post-acute care and long-term facilities, utilize EMR for notes
Qualifications: Board-certified APP, NP, or PA 1 year of experience caring for the geriatric population and post-acute care is a plus

Demands: This physician/APP role requires stamina in fast-paced healthcare: prolonged standing/walking (8-12 hrs/shift); rapid emergency movement (sprints, crowds); lifting/carrying 25-50 lbs equipment; pushing/pulling 100 lbs (beds/carts) with aid; frequent bending/stooping/kneeling for care; constant reaching/fine motor skills (suturing/IVs); occasional climbing/balancing; tolerating stressors (temps, noise, fluids, hazards) during irregular shifts/on-call/infection risks— accommodations for disabilities; varies by setting (clinic/ER).

Job Responsibilities

Caring and talented APP to cater to the medical needs of the seniors/adults
Monday-Friday, 8am-5PM or 9-6PM
Call One in 3 weekends per month, mostly tele
14-16 patients per day
Take care of the patient in post-acute care and long-term facilities

Qualifications

Qualification:

Board-certified APP, NP or PA
1 year Experience caring for the geriatric population and post acute care a plus

Working Hours

8am-5pm or 9-6pm
Can be Flexible hours
8 hour shifts
