Position Overview
We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality care to adult and geriatric patients across a variety of care settings. This role offers the opportunity to work in a collaborative environment focused on improving outcomes for patients in post-acute and long-term care.
Job Responsibilities
Provide comprehensive medical care to adult and geriatric populations
Practice across post-acute, skilled nursing, long-term care, and assisted living facilities
Perform patient assessments and manage acute and chronic conditions
Develop and implement individualized treatment plans
Prescribe medications and order/interpret diagnostic tests
Collaborate with nursing staff and consulting providers for coordinated care
Maintain accurate and timely clinical documentation
Participate in on-call rotation as required
Qualifications
Graduate of an accredited NP or PA-C program
Active or eligible for Washington state licensure
DEA, NPI, and prescriptive authority (or eligibility)
Board Certified or Board Eligible
Eligible to participate in Medicare and Medicaid programs

Working Hours
Competitive compensation: $110K – $160K
