Job description Nurse Practitioner - Skilled Nursing / Chronic Disease Management

Our client is seeking a Nurse Practitioner to travel to 1-2 Nursing Homes per day around New Castle, PA.

Training is available for candidates with strong primary care / chronic disease management experience.

Schedule:
  • Full Time: Monday – Friday, flexible start/end time
  • No weekends
  • On-call once a month (can be done telephonically/virtually from home and it is rare to receive calls)
Compensation: $110,000 - $125,000/yr base salary for the first 3 months (half of the time is training), then it switches to an encounter-based/RVU structure, where you can earn anywhere from $120k-$160k
  • 4 weeks PTO
  • 401k with company match (100% of contributions on the first 3% and 50% on contributions from 3-5%)
  • CME yearly allowance of $3000 and 3 CME days per year

Nurse Practitioner Responsibilities:
  • Perform comprehensive physical evaluations for an Adult Geriatric population
  • Provide symptom management
  • Acts as a medical liaison between the facility and others in the medical community such as specialists and hospital staff
  • Work collaboratively with an interdisciplinary team to ensure continuity of care
  • Prescribe all necessary medications and treatments
Qualifications:
  • Master’s Degree in Nursing
  • Licensed Family or Adult Geriatric Nurse Practitioner in the state of Pennsylvania
  • Ability to obtain DEA license
  • 1+ year of experience as a Nurse Practitioner within Primary Care, Skilled Nursing, Home Health or similar setting
