We are seeking an experienced Nurse Practitioner (NP) for a locum tenens opportunity providing geriatric-focused primary care in the San Fernando Valley, California. This role is ideal for providers who enjoy autonomous practice, relationship-based care, and a predictable weekday schedule.
The position is home-based, with providers seeing patients in residential homes and care facilities, supported by a structured clinical model and virtual physician oversight.

• Location: San Fernando Valley, CA
• Start Date: ASAP
• Duration: Initial 6-month assignment with extension potential

Schedule & Workload
• Monday–Friday
• Daytime hours
• No nights, weekends, or call
• Average 5–7 patients per day
• Assigned patient panel once ramped (approximately 70–90 patients)

• Home-based and facility-based geriatric care
• Focus on chronic disease management, preventative care, and continuity
• EMR system in place
• Physician support available virtually/by phone
• Structured workflows designed for quality over volume

Candidate Requirements
• Active California NP license
• Board Certified Nurse Practitioner
• DEA and BLS required
• Minimum 1 year of experience in geriatrics, primary care, home health, or similar setting
• Comfort working independently in a mobile/home-based model
• Clean professional and licensure history
