Position Overview
Seeking a full-time Nurse Practitioner to provide medical care to patients in a skilled nursing facility setting. The role focuses on subacute and long-term care patients, chronic disease management, and coordination of care with interdisciplinary teams.
Clinical Responsibilities

Manage subacute and long-term care patients in a SNF setting

Perform initial evaluations, daily follow-ups, and discharge assessments

Conduct history and physical exams

Diagnose and manage acute and chronic medical conditions
-
Order and interpret labs, imaging, and diagnostic studies

Develop and adjust treatment plans

Coordinate care with nursing staff, therapy teams, physicians, and families

Complete timely and accurate clinical documentation in the EMR
Schedule

Full-time schedule

Monday–Friday coverage typical

Daytime hours
-
No overnight call (or minimal call, if applicable)
Patient Load

Census consistent with standard SNF NP coverage models

Provider works independently with physician oversight as required

Candidate Requirements

Nurse Practitioner (NP)

Active Texas NP license
-
National NP board certification
-
Experience in SNF, post-acute care, geriatrics, or internal medicine preferred

Ability to work independently in a SNF environment

Strong clinical assessment and documentation skills
