Position Overview

Seeking a full-time Nurse Practitioner to provide medical care to patients in a skilled nursing facility setting. The role focuses on subacute and long-term care patients, chronic disease management, and coordination of care with interdisciplinary teams.

Clinical Responsibilities

  • Manage subacute and long-term care patients in a SNF setting

  • Perform initial evaluations, daily follow-ups, and discharge assessments

  • Conduct history and physical exams

  • Diagnose and manage acute and chronic medical conditions

  • Order and interpret labs, imaging, and diagnostic studies

  • Develop and adjust treatment plans

  • Coordinate care with nursing staff, therapy teams, physicians, and families

  • Complete timely and accurate clinical documentation in the EMR

Schedule

  • Full-time schedule

  • Monday–Friday coverage typical

  • Daytime hours

  • No overnight call (or minimal call, if applicable)

Patient Load

  • Census consistent with standard SNF NP coverage models

  • Provider works independently with physician oversight as required

Candidate Requirements

  • Nurse Practitioner (NP)

  • Active Texas NP license

  • National NP board certification

  • Experience in SNF, post-acute care, geriatrics, or internal medicine preferred

  • Ability to work independently in a SNF environment

  • Strong clinical assessment and documentation skills

  • Ability to meet credentialing and onboarding requirements
