A full-time hybrid NP role providing high-quality, patient-centered primary care to seniors in assisted living communities. Enjoy excellent compensation, meaningful work, and true work-life balance — no nights, no weekends ✨
Job Responsibilities
• Perform comprehensive health assessments 🩺
• Manage chronic and acute medical conditions
• Review and prescribe medications within NP scope
• Order and interpret labs and diagnostics
• Develop individualized care plans with care teams
• Educate patients and families on health & wellness
• Maintain accurate EMR documentation
Qualifications
• Master’s degree from an accredited NP program 🎓
• Active New York State NP license & DEA
• Eligible for Medicare/Medicaid billing
• Minimum 3 years of clinical experience (geriatrics preferred)
Working Hours
• Full-Time
• Day shifts only ☀️
• No nights | No weekends
