Job Responsibilities
• Perform comprehensive geriatric assessments and exams
• Manage chronic and acute conditions
• Develop and implement personalized care plans
• Prescribe medications and review treatment plans
• Order and interpret labs and diagnostics
• Educate patients and families
• Document care in eClinicalWorks
• Collaborate with interdisciplinary care teams
Qualifications
• Master’s Degree in Nursing
• Active New York State NP license & DEA
• Eligible for Medicare/Medicaid billing
• Minimum 3 years of clinical experience
• Geriatric or long-term care experience preferred
Working Hours
🕘 Full-Time | Monday–Friday
🚫 No Nights | 🚫 No Weekends
🏥 Hybrid (On-site + Telehealth)
