Job Responsibilities
• Perform comprehensive health assessments and exams
• Manage chronic and acute conditions
• Develop individualized care plans with the care team
• Order and interpret labs and diagnostic tests
• Prescribe medications within NP scope
• Educate patients and families on wellness and prevention
• Maintain accurate documentation in EMR
• Participate in quality improvement initiatives
Qualifications
• Master’s degree in Nursing (NP program)
• Active New York State NP license
• DEA registration
• Eligible for Medicare/Medicaid billing
• Minimum 3 years of clinical experience (geriatrics preferred)
Working Hours
🕘 Day shifts only
