Provide compassionate, patient-centered care to seniors in assisted living communities through a hybrid model (on-site visits + telehealth). Provide compassionate, patient-centered care to seniors in assisted living communities through a hybrid model (on-site visits + telehealth). Job Responsibilities
• Conduct comprehensive health assessments and physical exams
• Manage chronic and acute conditions in geriatric patients
• Review and prescribe medications within NP scope
• Order and interpret labs and diagnostic tests
• Develop individualized care plans with interdisciplinary teams
• Educate residents and families on preventive care
• Maintain accurate documentation in EMR systems
• Participate in quality improvement initiatives
Qualifications
• Master’s degree from an accredited Nurse Practitioner program
• Active New York State NP license
• Active DEA registration
• Eligible for Medicare/Medicaid billing
• Minimum 3 years of clinical experience (geriatrics preferred)
Working Hours
• Full-time
• Day shift only
• No nights | No weekends
