Position Overview:
We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality care to residents at a skilled nursing and rehabilitation facility. This role focuses on primary and preventive care, chronic disease management, and managing residents with acute changes in condition. The ideal candidate is comfortable working in an interdisciplinary team and is dedicated to improving the health and quality of life for long-term care patients.

Job Responsibilities
Manage residents experiencing acute changes in condition
Conduct advanced care planning and chronic care management
Prescribe medications, including controlled substances, in accordance with Pennsylvania law
Order, review, and follow up on diagnostic testing and treatments
Communicate regularly with nursing staff regarding patient condition and care plans
Collaborate with primary care physicians and interdisciplinary teams
Support patient and family education as needed
Qualifications
Board Certification
Active DEA license
Clinical experience in adult, family, or geriatric specialties
Experience working in skilled nursing or long-term care settings
Ability to work with an EMR system
Skilled in managing patients with advanced illness and multiple chronic conditions
Strong ability to work collaboratively within an interdisciplinary care team
Working Hours
No on-call or weekends!
