🏥 Facility Setting
Community-Based Geriatric Care Program | Outpatient Clinic + Home-Based Care | Interdisciplinary Care Model

🩺 What You’ll Be Doing

• Perform comprehensive medical assessments

• Develop and manage individualized care plans

• Evaluate and treat acute and chronic conditions

• Conduct home visits as clinically needed

• Coordinate care across the full continuum of services

• Support discharge planning and transitions of care

• Participate in provider on-call rotation

• Help develop and review clinical policies and procedures

• Full-time weekday schedule

• On-call rotation as part of the provider team

• Board-certified NP (ANCC or AANP)
