🩺 Key Responsibilities
- Perform comprehensive medical assessments
- Develop and implement individualized care plans
- Evaluate and treat acute illnesses
- Participate in discharge planning
- Conduct home visits as clinically indicated 🏠
- Coordinate care across medical, social, and supportive services
- Assist with development and review of clinical policies & procedures
- Participate in provider on-call rotation

- Active Massachusetts NP license
- DEA Registration
- BLS certification
