We are seeking a compassionate and dedicated Nurse Practitioner to join our interdisciplinary team in West Springfield, Massachusetts. This full-time position provides the opportunity to deliver high-quality, community-based care to frail and elderly adults within the PACE (Program of All-Inclusive Care for the Elderly) program. The successful candidate will be an integral part of our mission to promote independence, dignity, and quality of life for our elderly population.

General Responsibilities:

As a Nurse Practitioner in the Geriatric Medicine department, your responsibilities will include:

  • Comprehensive Medical Assessments: Perform in-depth evaluations of elderly patients to determine their medical needs.

  • Develop Individualized Care Plans: Create tailored care strategies for each patient to ensure optimal outcomes.

  • Acute Illness Evaluation and Treatment: Diagnose and manage acute medical conditions in frail and elderly individuals.

  • Home Visits: Provide care in patients’ homes when clinically appropriate.

  • Care Coordination: Ensure seamless care coordination across the full spectrum of services offered by PACE, including follow-up and discharge planning.

  • Policy Review and Development: Contribute to the creation and ongoing review of clinical policies and procedures to maintain the highest standards of care.

  • On-Call Rotation: Participate in the provider on-call rotation (specific times not yet defined).

Qualifications & Requirements:

  • Board Certified Nurse Practitioner (ANCC or AANP)Required

  • Active Massachusetts NP LicenseRequired

  • DEA RegistrationRequired

  • CDS CertificationRequired

  • BLS CertificationRequired

  • Copy of Most Recent NPDB Report (within 30 to 60 days active) – Required (no exemptions)

  • Experience with Frail or Elderly PopulationRequired

  • Ability to Travel Within Service AreaRequired
