We are seeking a full-time Nurse Practitioner to join an interdisciplinary PACE (Program of All-Inclusive Care for the Elderly) team in West Springfield, Massachusetts. This role offers a unique opportunity to provide comprehensive, patient-centered care to frail and elderly adults while supporting their independence, dignity, and quality of life.
PACE is designed for adults age 55+ who qualify for nursing home–level care but wish to remain safely in their homes and communities. Through a fully integrated model, the program delivers the complete continuum of medical and supportive services—allowing participants to live independently for as long as possible.
What You’ll Do

Perform comprehensive medical assessments for PACE participants

Develop, implement, and manage individualized care plans

Evaluate and treat acute and chronic conditions

Participate in discharge planning and care transitions

Conduct home visits as clinically indicated

Collaborate closely with an interdisciplinary care team

Coordinate care across the full continuum of PACE services

Assist with development and review of clinical policies and procedures

Participate in provider on-call rotation

Required Qualifications
-
Board Certified Nurse Practitioner (ANCC or AANP)
-
Active MA License
-
DEA Registration
-
CDS/Controlled Substance Certification
-
BLS Certification
-
Experience working with frail and/or elderly populations
-
Ability to travel within the service area
