We are seeking a full-time Nurse Practitioner to join an interdisciplinary PACE (Program of All-Inclusive Care for the Elderly) team in West Springfield, Massachusetts. This role offers a unique opportunity to provide comprehensive, patient-centered care to frail and elderly adults while supporting their independence, dignity, and quality of life.

PACE is designed for adults age 55+ who qualify for nursing home–level care but wish to remain safely in their homes and communities. Through a fully integrated model, the program delivers the complete continuum of medical and supportive services—allowing participants to live independently for as long as possible.

What You’ll Do

  • Perform comprehensive medical assessments for PACE participants

  • Develop, implement, and manage individualized care plans

  • Evaluate and treat acute and chronic conditions

  • Participate in discharge planning and care transitions

  • Conduct home visits as clinically indicated

  • Collaborate closely with an interdisciplinary care team

  • Coordinate care across the full continuum of PACE services

  • Assist with development and review of clinical policies and procedures

  • Participate in provider on-call rotation

Required Qualifications

  • Board Certified Nurse Practitioner (ANCC or AANP)

  • Active MA License

  • DEA Registration

  • CDS/Controlled Substance Certification

  • BLS Certification

  • Experience working with frail and/or elderly populations

  • Ability to travel within the service area
