• Perform comprehensive medical assessments

• Develop and manage individualized care plans

• Evaluate and treat acute and chronic conditions

• Coordinate care across the full continuum of services

• Participate in discharge planning

• Conduct home visits within the service area

• Board Certified Nurse Practitioner (AANP or ANCC)

• Active Massachusetts NP license with prescriptive authority
