Job description

Job description

This organization is transforming mental health care through a virtual-first, team-based care model serving older adults with serious mental illness, substance use disorders, and dementia. Care is holistic, compassionate, and community-oriented—focused on treating the whole person, not just symptoms.

The Psychiatric Mental Health Nurse Practitioner will provide comprehensive psychiatric care, collaborate closely with an interdisciplinary team, coordinate with primary care providers, and support patients and families through complex clinical needs. This role includes crisis management, care coordination, and active participation in a fast-growing, innovative startup environment.

Ideal candidates are patient-centered, tech-savvy, adaptable, and passionate about improving access and outcomes in mental health care. This is a fully remote opportunity offering meaningful impact and collaborative clinical support.

Job Responsibilities

Conduct initial psychiatric evaluations and follow-up visits for patients
Provide compassionate, patient-centered psychiatric care with empathy, patience, and professionalism
Manage crisis situations and provide appropriate interventions for high-acuity patients
Prescribe and manage psychiatric medications, including for complex and geriatric populations
Collaborate with families, caregivers, and interdisciplinary care teams to support comprehensive treatment planning
Make appropriate referrals to psychotherapy, care management, and additional support services
Participate in regular weekly supervision with a supervising physician to review caseloads and clinical decision-making
Engage with clinical quality metrics, audits, and performance reviews, and respond to recommendations for improvement
Maintain timely documentation and demonstrate responsiveness to patient calls, messages, and medication requests
Communicate clearly and effectively with patients, peers, managers, and leadership
Support pilot programs and assist colleagues as needed to align with organizational goals
Participate in after-hours on-call coverage by phone approximately one week every 3–4 months (light call volume, averaging 0–2 calls per day)

Qualifications

Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP) with an active, unrestricted license and DEA to practice in Florida, Texas, Indiana, Arizona, Illinois, and additional states as needed
Credentialed with national payers (preferred)
Minimum of 3–5 years of experience as a Nurse Practitioner in acute psychiatric settings, including emergency departments, urgent care, inpatient units, intensive outpatient programs, ACT teams, nursing homes, and outpatient mental health clinics
Clinical experience treating older adult populations strongly preferred
Demonstrated experience managing complex patients with serious mental illness (SMI), substance use disorders (SUD), dementia, and co-occurring psychiatric and medical conditions
Strong clinical judgment and psychiatric knowledge base, including crisis management, diagnostic formulation, and geriatric psychopharmacology
Experience managing patients with significant medical comorbidities and complex care needs, including social determinants of health
Previous experience providing virtual psychiatric care or telehealth services required
Experience in managed care, collaborative care, or value-based care models preferred
Strong computer and technology skills, including fluency with virtual platforms and Google Workspace applications
Comfortable working in a fast-paced startup environment, with flexibility and adaptability to change

Working Hours

Monday - Friday, 9am - 5pm
On call via phone for 1 week every 3-4 months as needed with very light call volume (0-2 calls / shift)
