Job Responsibilities
• On-site primary care role in assisted living communities
• No nights, no weekends, no on-call
• Supportive team environment with focus on quality geriatric care
• Final office location to be confirmed at interview (multiple sites opening)
Qualifications
• Manage chronic and acute medical conditions
• Conduct medication reviews and reconciliation
• Develop individualized care plans with interdisciplinary team
• Order and interpret labs and diagnostic tests
• Prescribe medications within NP scope of practice
• Educate residents and families
• Maintain accurate documentation in eClinicalWorks
• Participate in quality improvement initiatives
Working Hours
• Part time: Flexible hours (candidate choice)
• AM shifts preferred
• On-site only (not remote)
• No nights, no weekends
