Nurse Practitioner Community-Based Care
Location: Arizona (Commutable to Maricopa County)
We are seeking a Nurse Practitioner to provide comprehensive primary and acute care to patients in Assisted Living, Memory Care, Hospice, and Group Home settings throughout Maricopa County. This is a weekday role with no nights, weekends, or call. Daily patient volume varies based on acuity and visit type (intake vs. follow-up).
Responsibilities
Provide primary and acute care to patients in community-based settings
Order, interpret, and evaluate diagnostic tests
Deliver patient education, counseling, and disease prevention guidance
Collaborate with an interdisciplinary care team
Develop, document, and update individualized care plans
Prescribe medications and treatments within scope of practice
Requirements
Master of Science in Nursing (MSN)
Minimum of one year of Nurse Practitioner experience, or equivalent clinical background
Current, unrestricted Arizona RN and NP licenses
Compensation & Benefits
Salary up to $145,000 annually

This position requires local travel within Maricopa County to Assisted Living, Memory Care, Hospice, and Group Home settings.
