Job description Provide on-site primary care to seniors in assisted living communities with no nights, no weekends, and flexible scheduling. Role focuses on patient-centered care, chronic disease management, wellness assessments, and meaningful long-term patient relationships. Compensation ranges from $60,000–$120,000 depending on schedule and experience, offering both W2 and 1099 options.

Job Responsibilities

Perform full health assessments, manage acute and chronic conditions, prescribe medications, order and interpret diagnostics, coordinate care plans, document in eClinicalWorks, educate residents and families, and participate in quality improvement initiatives.

Qualifications

Master’s degree in Nursing; active NY NP license; active DEA; minimum 3 years of clinical experience; Medicare/Medicaid billing eligibility; geriatric experience preferred.

Working Hours

Part-time or full-time; no nights; no weekends; on-site only.

Skills

Strong clinical assessment abilitygeriatric care knowledge, communication skills, compassionate bedside manner, medication management, diagnostic interpretation, and EMR proficiency (eClinicalWorks).
