Job description
Flexible full-time or part-time role providing primary care to residents in assisted living communities. No nights, no weekends, and no burnout. Compensation ranges from $60,000–$120,000 per year with W2 and 1099 options. Strong emphasis on work–life balance, supportive team environment, and meaningful patient relationships. On-site role across New York communities.
Job Responsibilities
Develop individualized care plans with interdisciplinary teams
Manage chronic and acute medical conditions
Order and interpret labs and diagnostic testing
Prescribe medications within NP scope
Educate residents and families on wellness and independence
Maintain accurate documentation in eClinicalWorks
Participate in quality improvement initiatives
Qualifications
Active New York NP license and DEA registration
Eligible for Medicare/Medicaid billing
Minimum 3 years of clinical experience (geriatric/LTC preferred)
Strong communication, empathy, and patient-centered care approach
Working Hours
Flexible scheduling
On-site only (not remote)
No nights and no weekends
