Job description

Job description Seeking a Spanish speaking Nurse Practitioner to provide effective and equitable value-based primary care to Medicare patients in the Bronx in NY.

Opportunity Details:

  • Full-time, Mon - Fri, 8am-5pm
  • 13-16 patients per day
  • Geriatric population
  • Fluency in Spanish is required


Qualifications:

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required
  • National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
  • Experience in primary care: Internal medicine, geriatrics, or family medicine preferred
  • Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).


Compensation/Benefits:

  • Competitive Salary - $105,000-$148,500* *Wider range depending on experience
  • Quarterly bonus based on quality metrics
