Nurse Practitioner – Long-Term Care / Skilled Nursing Facilities (Shippensburg and Clearfield, Pennsylvania)
Location: Shippensburg or Clearfield Pennsylvania
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (40 hours/week)
Compensation: $115,000 – $130,000 annually (FTE, W2 with benefits)
Position Summary
We are seeking an experienced and compassionate Nurse Practitioner to provide primary and preventive care to long-term care patients residing in skilled nursing facilities. In this role, you will manage residents with acute changes in condition, oversee chronic care management, and collaborate closely with nursing staff and primary care physicians to ensure high-quality, patient-centered care.
This role is ideal for an NP who is experienced in adult, family, or geriatric medicine and comfortable working with patients who have advanced illness and multiple chronic conditions.
Key Responsibilities

Provide primary and preventive care to residents in skilled nursing facilities.

Evaluate and manage residents experiencing acute changes in condition.

Lead and participate in advanced care planning and chronic disease management.

Prescribe medications, including controlled substances, in accordance with Pennsylvania law.

Order, review, and follow up on diagnostic testing, labs, and treatment plans.

Communicate regularly with facility nursing staff to discuss changes in condition and necessary care plan updates.

Collaborate with the primary care physician and multidisciplinary care teams.

Document clinical encounters and care plans accurately within the EMR system.
Required Qualifications

Active, unrestricted Nurse Practitioner license in the state of Pennsylvania

Board certification (FNP, ANP, AGNP, etc.)

Active DEA license
-
Clinical background in adult, family, or geriatric medicine

Experience working in a long-term care or skilled nursing setting

Strong EMR documentation skills

Ability to manage patients with advanced illness and multiple chronic conditions
