Geriatric Nurse Practitioner opening in Rutland, VT - 4-day work week w/flexible hours!

Seeking an experienced Nurse Practitioner to join our dedicated team delivering high-quality care to older adults in the greater Rutland, Vermont area.


Details:

• Responsibilities include conducting annual wellness visits, along with other routine and specialized assessments, to ensure comprehensive care for older adults
• 3-5 years Primary Care Practice Experience or geriatric setting preferred

Essential functions include though are not limited to:

• Obtains complete medical history and physical examination on patients.
• Reviews medical records
• Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures
• Establishes presumptive diagnosis
• Orders appropriate laboratory and diagnostic procedures
• Develops a therapeutic plan for patient
• Sutures wounds
• Prepares written prescription orders for drugs and controlled substances
• Instructs patients and family regarding medications and treatment instructions
• Provides recommendations regarding health and illness prevention
• Recommend community resource to meet patient/family needs
• Performs independent evaluations and treatment procedures in emergency situations
• Provides monitoring and continuity of care between physician visits
• Maintains patient confidentiality
