The Role

As a Primary Care Nurse Practitioner, you’ll practice in a value-based care model focused on quality over volume. You’ll manage a patient panel of older adults living with chronic and complex conditions, supported by a dedicated care team.

Practice Highlights:

  • 100% outpatient practice

  • Patient population: seniors with 3–5 chronic conditions (e.g., heart disease, diabetes, hypertension)

  • Average of 10–12 patients per day

  • Patient panel capped at ~650

  • Dedicated support team including MA, scheduler, and referral specialist

Qualifications

- Licensed Nurse Practitioner
- Minimum 2 years of experience in outpatient primary care
- Geriatric experience a plus, but not required
- Active, unrestricted state license to practice medicine
- Current DEA registration

Working Hours

- Monday–Friday, 8:00 a.m.–5:00 p.m. schedule
- Shared call schedule (e.g., 1:32 if 32 providers in the market)

Skills

- Proficiency in office procedures including suturing, joint injections, skin biopsies, and wound care
