Job Responsibilities
In this flexible and patient-centered role, you will:
โ Provide primary care services to geriatric patients in post-acute rehab and assisted living settings
โ Manage a daily caseload of approximately 6โ12 patients, focusing on quality over quantity
โ Conduct patient rounds independently, with the flexibility to chart from home
โ Diagnose and treat a range of acute and chronic conditions in collaboration with facility staff
โ Coordinate with specialists and care teams, including physical therapy, nursing, and case management
โ Develop and update personalized care plans that align with patient goals and clinical best practices
โ Document all patient interactions accurately in Epic EMR
โ Respond to emergent needs within the facility and assist when required in urgent clinical situations
โ Build strong relationships with patients and families to support long-term wellness
โ Participate in occasional shadowing or onboarding support for new clinicians as needed
Qualifications
โ Licensed Nurse Practitioner (NP) or Physician Associate (PA)
โ Active, unencumbered license in the state of Oregon (or ability to obtain)
โ Board certified or board eligible
โ New grads welcome! โ strong interest in geriatrics is a must
โ Excellent communication and interpersonal skills
โ Comfortable working independently and managing daily workflow
โ Compassionate approach to geriatric care with an understanding of the unique challenges faced by aging patients
โ Proficient in electronic medical records (EMR), preferably Epic
โ Ability to respond effectively in urgent or high-pressure situations within a facility setting
โ A collaborative mindset and desire to work as part of a multidisciplinary team
Working Hours
๐ Typical Schedule: Around 8:00 AM โ 5:00 PM
๐ผ Flexible Hours: You can round on patients and chart from home
