A well-established Program of All-Inclusive Care for the Elderly (PACE) is seeking a Locum Nurse Practitioner (NP) to provide primary medical coverage for an ongoing assignment, with the potential to extend. This is a rewarding opportunity to work with a collaborative team delivering compassionate care to frail and elderly participants in a variety of settings.
What You’ll Do:
🩺 Provide primary care in collaboration with physicians
📝 Conduct initial, semi-annual, and annual assessments, implement care plans
🏥 Assist with discharge planning for acute and long-term placements
💊 Evaluate and treat participants during acute illness, prescribe medications/therapies
🚗 Make nursing home or home visits as needed
💼 Help develop clinical policies, procedures, and standards of care
📚 Train and support staff on medical and quality assurance issues
Requirements:
🎓 Master’s degree in nursing – REQUIRED
📜 Active PA License – REQUIRED
💊 BLS, CDS, DEA – REQUIRED
✅ Board Certified by ANCC or AANP – REQUIRED
