Details:
- Setting: hybrid remote and in-patients' homes role
- Location: Harrisburg/Hershey, PA
- Schedule: Mon-Fri (8a-5p includes travel time for in-home visits)
- Duties:
- APP's serve multiple serious conditions like CKD and/or ESRD/ESKD
- Conducts assessments including annual wellness exams
- Counsels and educates patients and their families about programs
- Responsible for coordination of care with primary care providers
- Part of the Integrated care team working with RN and Social Worker
- Prescribe medications, order tests, collaborate with Monogram physician
- Follow up and evaluate patient's status
Required:
- NP, FNP, AGNP, ACAGNP with Active PA license
- Board Certified- ANCC or AANP
- 2 or more years of direct patient care
- DEA, BLS
Compensation:
- Discussed by phone
- Receive a paycheck every Friday
- Mileage reimburseed
- 100% of malpractice insurance covered
