We are seeking a Locum Tenens Nurse Practitioner - Geriatric Medicine in Pennsylvania.

Position Details:

Setting: PACE (Program of All-Inclusive Care for the Elderly)

Responsibilities:
- Provide primary medical management in collaboration with PACE Physicians
- Perform assessments and develop care plans for participants
- Evaluate and treat participant physical complaints
- Provide education to participants and caregivers
- Function as part of the Interdisciplinary Team (IDT)
- Provide oversight of the PACE Clinic
- Perform rounds in contracted nursing homes
- Document participant conditions and update medical records

On-Call/Weekend Requirements:
- On-call every third week (Monday – Friday, 5:00 PM to 8:30 AM; Saturday and Sunday all day)

EMR System: Epic
Requirements:
- Active PA license – Required
- ANCC/AANP Board Certified – Required
- BLS Certification – Required
- CDS Certification & DEA Registration – Required
- 5-7 years of NP experience, preferably in long-term care, with at least 1 year working with the frail/elderly population – Required
- Clean/Clear malpractice history – Highly Preferred
