LOCATION:
- New York float position
- greater Syracuse area

The Float NP will travel approximately 3 weeks out of every month, providing coverage at SNFs in Syracuse and the surrounding areas. All travel expenses are covered by the company.


COMPANY PROFILE:
This organization is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for long-term nursing home residents. The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. They are strongly focused on improving their members' physical, mental, and social well-being through personalized clinical care and therapies. They are currently hiring due to growth.


POSITION DESCRIPTION:
The Float NP will provide as-needed coverage to geriatric patients in SNFs. The Float NP will not be permanently assigned to one facility, but will float within the designated geographic area for purposes of vacation coverage, sick leave coverage, onboarding new buildings, etc. The exact travel and schedule parameters will be mutually agreed upon by the company and the provider at the time of hire.

The Float NP will be responsible for:

- effectively medically managing enrolled SNF patients
- conducting comprehensive assessments
- developing and delivering individualized plans of care
- attending facility care team meetings
- communicating with family members and caregivers
- ensuring the care team understands the member’s goals of care and needs

As part of the clinical team, the Float NP will be supported by the Clinical Manager and local Medical Director.

The organization uses an EHR.


REQUIREMENTS:
- New York NP license
- DEA
- 1+ year advanced practice experience
- 1+ year geriatric experience as an advanced practice provider
- some previous experience in SNFs, LTC facilities, etc.
