Seeking a licensed Nurse Practitioner to join Skilled Nursing Facility Service Line that provides primary healthcare to our geriatric population residing in Skilled Nursing Facilities in the greater Rutland Area.

Our SNF Nurse Practitioners perform selective medical services through a collaborative agreement with a supervising MD/DO and must enjoy working with a dedicated team of highly motivated professionals who are committed to providing an integrated, community-based approach to care. Nursing Home Providers examine and treat patients independently and in collaboration with other health Care Professionals in a nursing home/SNF setting. They ensure proper illness and injury care and disease prevention, diagnosis, treatment, and recovery.

Qualifications

Advance Practice Registered Nurse (APRN), State Licensed with experience in primary healthcare; BLS certified
