- $120,000 to $135,000+ Total Income

This organization is a leading provider of integrated medical services caring for individuals from post-hospitalization to home in various settings ranging from skilled nursing facilities (SNFs), assisted living facilities (ALFs), independent living facilities (ILFs), group homes, and private residences. They bring quality medical care to the geriatric population, as well as people with complex conditions, special needs, and with intellectual and/or developmental disabilities. Their providers make a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve. They are currently hiring due to growth in their Terre Haute market.
POSITION DESCRIPTION:
Serve as the primary care provider for a dedicated geriatric patient panel.
The practice believes strongly in proactive medicine, with a clinical focus on:
- preventative care
- chronic disease management
- medication management
- routine geriatric primary care
Providers typically see patients once a month, but this is adjusted per provider request based on patient needs.
Providers typically travel to 2 buildings each day and see 10 to 11 patients, for a total of 50 to 55 visits per week. This role is based in ALFs with some house call patients.
The practice uses an EHR on a mobile-based platform specialized to this type of setting.
Providers share on-call responsibilities. The current rotation is 1 week every 4-5 months. On-call is phone triage only and volume is light.
REQUIREMENTS:
- Indiana NP license
- DEA
- 6+ months of advanced practice experience
- previous experience in home care, geriatrics, etc.
