Job description Dell Medical School in conjunction with Dell Children's Medical Center of Central Texas (DCMCCT), is seeking Advanced Practice Nurses (Nurse Practitioner or Physician Assistant) with cardiac critical care credentials to be part of the Texas Center for Pediatric and Congenital Heart Disease, working collaboratively with a multidisciplinary team to serve the growing need of the regional pediatric population, delivering world class pediatric cardiac critical care.

Job Responsibilities

Independently bills and provides clinical care and assesses new and follow up patient status by obtaining health history through patient/family interviews via face-to-face visit or televisits, and chart reviews. Independently see appropriate clinic patients including but not limited to CRC screening or surveillance. Patients may also be seen in conjunction with faculty physician.

Orders appropriate diagnostic and screening tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.
Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws.

Educates patients regarding health promotion/illness prevention, medications and treatments.

Recommends appropriate community resources to meet patient/family needs.

Communicates appropriate case management information to other professionals and community agencies.

Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures. Acts as a resource and a teacher for students in nursing and NP school.

Acts as a resource to RN and MA staff to answer questions, triage patients, and ensure appropriate patient scheduling, including those patients direct to procedure. Complies with patient confidentiality requirements.

Provides monitoring and continuity of care between visits according to treatment plan including triaging patient calls and electronic communications. Collaborates with multidisciplinary team members by making appropriate referrals.

Perform currently available office-based procedures, including gastric mapping, FibroScan and manometries in the GI clinic. Contribute to the development of other office-based procedures as the clinic expands.

Ensures goals and objectives for high value care are met. Participates in research projects in the clinic which may include consenting patients, updating databases and helping with protocols, as appropriate.

Provides regular inpatient hospital coverage at Ascension Seton central Austin facilities, as deemed necessary.

Qualifications

Required Qualifications:
Currently licensed and certified Nurse Practitioner or Physician's Assistant in the state of Texas and be in good standing. Graduate of licensed and accredited nursing or physician assistant school. Bachelor's degree. At least two (2) years of experience in busy outpatient clinical practice. Demonstrated excellent analytical skills, problem solving ability and strong critical thinking skills. Ability to work efficiently and flexibly in a fast-paced environment with minimal guidance. Excellent written and verbal communication skills. Outstanding organizational and time-management skills. Ability to address individuals and information with a high degree of sensitivity and confidentiality in various delicate, formal, or urgent situations. Ability to develop and maintain effective working relationships with physicians, staff, and patients. Must be comfortable and flexible with changing priorities and self-directed problem solving.

Preferred Qualifications:

Five (5) or more years of experience in a busy outpatient GI clinical practice. Experience in liver and/or IBD patients. Working knowledge of and/or experience with Value-Based Healthcare Delivery models. Experience delivering care through a multidisciplinary team. Proficiency using Athena electronic medical records. Experience tracking, measuring, and using patient reported outcomes in developing treatment plans for patients. Bilingual in English and Spanish.
