Duties & Responsibilities
- Provide comprehensive primary care to elderly patients in clinic and community settings.
- Perform home visits to assisted living, SNF, and personal residences.
- Collaborate with interdisciplinary team (11 members) to coordinate patient care.
- Manage patient care via Epic EMR.
- Participate in PACE program, focusing on holistic care for over-55 population.
