SCHEDULE
- Coverage Type: Outpatient
- Schedule Type: Clinic only
- Call Type: N/A
- Schedule (per week): M-F ( we will consider 3-4 days a week as well)
- Dates of Need: November start
- Shift Times: 8am-5pm
- Lunch N/A
- Provider Min # Shifts Required: 2 weeks per month minimum (ideally would like a provider who is full time)
- Guaranteed Hours per Day: 8
- Hours Worked for OT to apply: 9
- Rates to be Filled Out on Bid: Clinic only: Regular, Orientation, Overtime, Holiday
CREDENTIALING
- Current CV Must be uploaded to bid

ADDITIONAL DETAILS
This site has a combination of in-clinic and community patients who are seen in their home. For off-site visits, this will involve driving to the patient's residence which can be Assisted living, SNF, or personal residence where usually with a nurse from our team will join the provider for the visit. We serve exclusively adults over the age of 55 as a PACE program (Program of All Inclusive Care for the Elderly) which means you'll be working with a 11 person interdisciplinary team. Most patient management is handled through the EMR with other team members coordinating visits and care in the community.
