Position Summary:
We're seeking a Nurse Practitioner or Physician Assistant to provide comprehensive primary care to an adult and geriatric patient population in a community clinic setting.
Duties & Responsibilities:
- Provide primary care services to clinic patients, including physical examinations, ordering and interpreting labs/x-rays/diagnostic tests, and determining and implementing treatment plans
- Monitor treatment outcomes and adjust care plans as needed
- Deliver health education on wellness, hygiene, nutrition, sexual health, and medication use
- Proactively support patients and families in accessing care, serving as a resource for care coordination
- Participate in peer review and help set clinical standards for support staff
- Meet regularly with the care team to discuss clinical concerns and quality improvement opportunities; consult with supervising physician as needed
- Contribute to Best Practices initiatives, helping shape protocols and care guidelines
- Participate in developing and launching new clinic programs and initiatives
- Help identify training needs and support staff development
Requirements:
- Active, unrestricted license to practice as a Nurse Practitioner or Physician Assistant in the State of California, with no pending or prior disciplinary action
- NP candidates: eligibility for a 103 license, with interest in eventually obtaining a 104
- Current DEA registration and BLS certification
- Competency in general adult ambulatory care, family planning, adolescent health, and women's health
- Strong understanding of current medical, educational, and psychosocial intervention approaches
- Ability to perform clinical duties efficiently within established guidelines
- Strong communication skills across diverse cultural and ethnic communities; fluent written/spoken English required, bilingual Spanish preferred
- Experience in a community clinic or outpatient setting strongly preferred
- Familiarity with CHDP, FPACT, EWC, Medi-Cal, and Title X programs preferred
